What Does BBL Stand For?
BBL stands for Brazilian butt lift. It is the everyday name for buttock fat grafting: a cosmetic procedure in which a surgeon removes fat from areas such as the tummy, waist or thighs using liposuction, prepares it, and transfers it into the buttocks to add volume and improve shape. Despite the name, nothing is lifted — and the technique is not specifically Brazilian.

What does BBL stand for?
In cosmetic surgery, BBL is short for Brazilian butt lift. Surgeons increasingly use more precise names for the same operation: gluteal fat grafting or, in the UK, superficial gluteal lipofilling (SGL) — a term adopted by the British Association of Aesthetic Plastic Surgeons (BAAPS) to describe the safer, above-the-muscle technique now recommended.
The confusion is understandable. The procedure does not lift the buttock the way a surgical lift (with skin removal) would. Instead, it uses your own fat as a natural filler to change projection and proportions — which is why many patients choose it over implants.
Why is it called a Brazilian butt lift?
The name is marketing shorthand, not medicine. Fat grafting techniques were developed by surgeons in several countries over decades, and the association with Brazil owes more to 1990s media coverage and beauty trends than to any Brazilian surgical standard. The takeaway for patients: judge the operation by the surgeon, technique and facility — not by the branding.
What actually happens during a BBL
- Liposuction. Fat is removed from donor areas — commonly the abdomen, flanks, back or thighs — through small incisions.
- Preparation. The harvested fat is processed so that healthy fat cells can be re-injected.
- Transfer. The surgeon injects the fat into the buttocks in the layer above the muscle, shaping volume and symmetry as they go.
Because a BBL is really two procedures in one, the liposuction plan matters as much as the transfer itself: the same operation that adds volume behind also slims the waist or tummy in front.

How safe is a BBL now? What changed
The honest answer: BBL earned a poor safety reputation in the 2010s, and the profession responded by changing the technique. According to the BAAPS gluteal fat grafting safety review published in 美容外科杂志 (2023), the risk of death from fat embolism was once reported as around 1 in 2,500 procedures, with later estimates nearer 1 in 15,000 — a level the review describes as similar to tummy tuck surgery. The deaths investigated were linked to fat injected into the deep muscle, where it can enter large veins.
Two rules now define modern practice:
- Subcutaneous placement only. Fat goes into the layer above the muscle — never into it.
- Ultrasound guidance. The surgeon watches the cannula in real time to confirm it stays in the safe plane.
The same BAAPS review adds a caution every medical tourist should hear: complications are better controlled in properly regulated hospital settings, and patients treated at unregulated facilities — at home or abroad — face higher risk. A BBL remains real surgery with real risks — including infection, asymmetry, fat reabsorption, seroma and, rarely, fat embolism. Balanced information, an honest medical assessment and a regulated setting are the non-negotiables.
Who is — and is not — a candidate
You may be a good candidate if you are in good general health, at a stable weight, have enough donor fat to harvest, and want natural-looking reshaping rather than an exaggerated result. A BBL may not be suitable if you are very lean (too little donor fat), planning significant weight loss, have conditions that raise anaesthetic or clotting risk, or expect guaranteed outcomes — no responsible surgeon promises those. Suitability can only be confirmed by medical evaluation; some patients are better served by implants, or by no surgery at all.
