What is a gastric sleeve, and what to expect

The surgeon permanently removes the larger, outer part of the stomach, leaving a narrow, banana-shaped sleeve. Food still follows the normal route, with no rerouting, unlike a gastric bypass. The smaller stomach holds less, and many patients notice their appetite is quieter.

Who is a gastric sleeve right for — and who should wait?

A gastric sleeve often suits you if

  • Your BMI is 40 or more
  • Your BMI is 35 or more with type 2 diabetes, high blood pressure, sleep apnoea, fatty liver or joint disease
  • Your BMI is 30 to 35 with poorly controlled metabolic disease, assessed case by case
  • You are ready for lasting changes to how you eat, with lifelong supplements and follow-up

It may be better to wait or choose another option if

  • You have severe acid reflux, Barrett's oesophagus or a large hiatus hernia
  • You have active substance or alcohol misuse, an untreated eating disorder or uncontrolled psychiatric illness
  • You are pregnant, plan to conceive within about 18 months, or anaesthesia is unsafe for you

Lower thresholds may apply to people of Asian ancestry. Check your figure with our free BMI calculator and the bariatric surgery requirements checklist.

Step by step

How a sleeve gastrectomy is done, step by step

Preparation starts before you travel, often with a two-week low-calorie or liquid diet to shrink the liver. In Istanbul, blood tests, ECG, chest imaging and an ultrasound come first. If a result is outside a safe range, surgery is postponed rather than rushed.

  1. Anaesthesia

General anaesthesia, with leg compression devices against clots.

  2. Keyhole access

Four to five small incisions in the upper abdomen.

  3. Freeing

Blood vessels along the stomach's outer curve are sealed.

  4. Stapling

A stapler shapes a narrow sleeve around a calibration tube.

  5. Leak test

The staple line is checked and reinforced where needed.

  6. Closing

Small wounds closed, usually with dissolvable stitches.

Recovery

Gastric sleeve recovery timeline

Your diet moves through four textures over about six to eight weeks. Most people return to desk work in two to three weeks.

  1. Weeks 1–2Clear, then full liquids: water, broth, protein shakes and thin soups. Aim for about 1.5–2 litres a day in small sips.
  2. Weeks 3–4Purée: smooth fish, eggs, lentils and yoghurt, often a few tablespoons. Many return to desk work.Tiredness is normal early on
  3. Weeks 5–6Soft, fork-mashable foods. Eat slowly, chew well and stop at the first sign of fullness.
  4. Weeks 7–8 onwardsRegular textures: protein first, then vegetables, then small amounts of starch. Gym work returns from about 4–6 weeks, once your surgeon agrees.
  5. Months 6–18Weight loss is quickest in the first six months and usually slows by 12 to 18 months.

Lifelong vitamins and dietitian follow-up

You take a bariatric multivitamin every day for life, plus calcium with vitamin D, and vitamin B12 as advised. Iron is added if your levels fall.

A Vivid Clinic dietitian follows you remotely by video or messaging. Blood tests are repeated at around 3, 6 and 12 months, then yearly.

Choosing the right operation

Gastric sleeve vs Roux-en-Y gastric bypass

In short: a sleeve offers restriction plus hormonal change without rerouting and with simpler nutrition. A bypass is often preferred for reflux or long-standing diabetes.

Rerouting of the intestineNoYes

A mini gastric bypass (OAGB) uses one join instead of two. For lower BMIs, a gastric balloon or gastric botox is non-surgical, temporary and more modest. Our guide to the four types of bariatric surgery goes into more detail.

Safety

Risks of a gastric sleeve and how we reduce them

Every operation carries risk, and a gastric sleeve is no exception. A staple-line leak is uncommon but serious, which is why we keep you close to the hospital for the first week.

Most sleeve patients spend two to three hospital nights.Illustration

Possible effects

  • Staple-line leak or bleeding, occasionally needing further treatment
  • Blood clots in the legs or lungs
  • New or worse acid reflux, or narrowing of the sleeve
  • Nutritional deficiencies if supplements are missed
  • Gallstones during rapid weight loss, and weight regain in the long term

How we reduce them

  • Careful patient selection
  • Full pre-operative testing, with surgery postponed if needed
  • Accredited hospitals with intensive-care backup
  • Early walking, compression and blood-thinning injections
  • A conservative fly-home date and structured follow-up

FAQ

Frequently asked questions about gastric sleeve in Istanbul

How long do I need to stay in Istanbul for a gastric sleeve?

Plan on about seven days in Istanbul for a gastric sleeve. That covers a day of tests and consultation, surgery, two to three nights in hospital and a few days in your hotel so the team can check that you are drinking well, walking comfortably and free of warning signs before you fly. Some patients stay longer if they have diabetes, sleep apnoea or a longer journey home, and your surgeon confirms the date.

What BMI do I need for a gastric sleeve in Istanbul?

For a gastric sleeve in Istanbul, the mainstream starting point is a BMI of 40 or more, or 35 or more with an obesity-related condition such as type 2 diabetes, high blood pressure or sleep apnoea. International ASMBS/IFSO guidance also allows surgery to be considered at a BMI of 30 to 34.9 with metabolic disease. Our medical team reviews each case individually, so a BMI alone never decides it.

Which pre-operative tests are done before a gastric sleeve in Istanbul?

Before a gastric sleeve in Istanbul you will usually have a full blood count, biochemistry, clotting, blood sugar and HbA1c, thyroid tests and vitamin levels, plus an ECG, chest imaging and an abdominal ultrasound. Many patients also have an upper endoscopy to check for reflux, hiatus hernia or Helicobacter pylori. An anaesthetist reviews the results, and surgery goes ahead only when they are acceptable.

Is gastric sleeve in Istanbul a good choice if I already have acid reflux?

Not always. A sleeve can worsen existing reflux and, in some people, cause new reflux over the long term; the ten-year SLEEVEPASS trial found reflux more often after a sleeve than after a bypass. If you already take reflux medication or have a hiatus hernia or oesophagitis on endoscopy, your surgeon may recommend a Roux-en-Y gastric bypass instead, or a hernia repair at the same time.

How much weight will I lose after a gastric sleeve in Istanbul?

Most people lose weight fastest in the first six months and reach their lowest weight at around 12 to 18 months. In the SLEEVEPASS randomised trial, patients had lost about half of their excess weight at five years on average, with some regain later. Your result depends on your starting weight, health conditions, eating habits, activity and how closely you follow the dietitian's plan.

When can I go back to work and exercise after a gastric sleeve in Istanbul?

You walk within hours of waking and several times a day during your stay. Most people return to desk work in two to three weeks, usually during the purée stage. Gym work and heavier lifting usually return from about four to six weeks, once your surgeon agrees. On the flight home, wear compression stockings, drink little and often, and walk the aisle when you can.

Which vitamins will I need after a gastric sleeve in Istanbul?

After a gastric sleeve in Istanbul you take a bariatric multivitamin every day for life, plus calcium with vitamin D, and vitamin B12 as advised. Iron is added if your levels fall. Missed supplements can lead to deficiencies of iron, B12, folate, vitamin D and calcium. Blood tests at around 3, 6 and 12 months, then yearly, show whether your doses need adjusting, with your local doctor or through our team.
