Newly grafted fat needs a blood supply to survive. Prolonged pressure squeezes the small vessels that are forming, which is why sitting rules exist. Your surgeon may adjust these times to your case.

Fat that survives the first months behaves like normal body fat. Results are long lasting if your weight stays stable; weight gain or loss will change them.

Fat embolism is the most serious risk: fat enters a damaged vein, travels to the lungs and blocks blood flow. Large fat embolism is a medical emergency and has caused deaths. Sudden breathlessness, chest pain or confusion after BBL surgery need emergency care immediately.

A 2017 task-force survey estimated BBL mortality at roughly 1 in 3,000 (Mofid et al., Aesthetic Surgery Journal). After the safety recommendations, a 2020 survey reported around 1 in 15,000 (Rios & Gupta, Aesthetic Surgery Journal). The risk is lower, not zero. Expect a refined shape rather than a large increase in size. If there is too little fat for a meaningful result, your surgeon will say so honestly and discuss alternatives or a staged plan.
No. Ultrasound lets the surgeon see the cannula and confirm that fat is going into the subcutaneous layer, which supports safe technique. It does not replace the core rules: subcutaneous-only injection, correct cannula size and angle, and careful patient selection. No method removes every risk of surgery. Ask your surgeon at consultation whether ultrasound guidance will be used for you.
It varies between patients, so surgeons avoid promising a fixed percentage. Some of the transferred fat is reabsorbed in the first two to three months, and what remains after that is generally stable. Not smoking, avoiding pressure on the buttocks, keeping a stable weight and wearing your garment as advised all help. Some patients choose a smaller second session after at least six months.
